Three difficulties
Difficulties are chosen when you make a lobby.
Mild uses a Mezzo face. Smile More sets its reward multiplier to 0.6x, up from 0.5x, cuts the enemy count by 25%, removes the rule that innocents take 50% less damage, and forces the combined fake-icon plus wrong-color disguise. The difficulties article still also describes halved enemy damage, simpler bosses, a slower Eye of Enphoso, and a maximum rank of A. Those lines were not repeated in the Smile More Mild section, so confirm them in the lobby.
Smile uses a Giggler face. It is the default and pays the normal Bintar and experience.
Maniac uses a laughing Murpleck face. It was added with Smile More. Enemies act in a more complex way, and the run starts with the extra shop currency described in the Smile More guide. Detailed Maniac-only move lists from earlier drafts of the update page are not repeated here, because that page was rewritten once the update shipped.
Boss health scaling on the current changelog is final = base × (1 + 0.35 × (players − 1)), after the flat +50% base increase (+25% on Mild). The older articles used +50% per extra player. Plug in the base health you see for that boss; this site does not have a confirmed post-overhaul base for Gergro, Borgack, or Risio.
Modifiers worth knowing
Modifiers change the Bintar and experience multiplier. Smile More requires some multiplier progress, or a finished run, before you can enable them. Joining a lobby that already has them also works. A total multiplier of 1x or lower turns off achievements and map completion.
The modifier article's old multipliers are stale where Smile More restates them:

Smile More says more modifiers were added. Their names were not on the modifier article when it was read, so they are not invented here.
Hearty's old health examples (Giggler 150, Gergro 3750, and the rest) belong to the pre-overhaul health table. Do not paste them into a Maniac plan.
